A supportive step between hospital and community
The Community Transition Home (CTH) is a specialized setting operated in partnership with the Department of Opportunities & Social Development and the East Coast Forensic Hospital. It allows Mosaic to support adults who are transitioning from hospital into community living.
Support designed for transition
CTH offers a structured, supportive environment where participants can begin rebuilding daily routines, confidence, and independence. Staff work closely with each participant and their care team to provide guidance during this important transition period.
With a focus on safety, dignity, and personal progress, CTH helps participants prepare for the next stage of their recovery — whether that is moving into another Mosaic setting, connecting with a new service provider, or transitioning to independent living.
