Independent living with personalized support
Our Independent Living Support Services provide participants with the opportunity to live independently in their own apartments or homes, with individualized support tailored to their specific needs, goals, and choices.
A team behind you
Each participant is assigned an Mosaic support staff who works closely with them, their family, and their community care team to identify personal goals and determine the level of support needed. Support plans and service hours are flexible and adjusted as circumstances change.
The support you need
Participants may receive assistance with:
- Budgeting and money management
- Grocery shopping and meal planning
- Home maintenance
- Psychiatric and other medical concerns
- Community access and social skill building
- Problem-solving and crisis management, including access to a 24/7 emergency response service
Life at Mosaic
Beyond day-to-day support, participants are encouraged to take part in Mosaic Community Living activities such as movie outings, picnics, clubs, and holiday gatherings. You get to celebrate accomplishments and build connections along the way.
All of our staff are competent, dedicated professionals who work in accordance with Mosaic’s comprehensive Code of Ethics, ensuring respectful and ethical service delivery for every participant.
